Job Description

ChildNet, Inc. is a private, not-for-profit community-based care (CBC) agency servicing Broward County which is contracted by the State of Florida, Department of Children and Families, to provide case management support to abused, abandoned, and neglected children. We are nationally accredited by the Council on Accreditation (COA) and employ over 450 staff members.

ChildNet is in need of individuals interested in the career path of working to unite families in becoming a Certified Case Manager.

Becoming a Certified Case Manager is an opportunity for select individuals to help vulnerable families through reunification, adoption, or independent living in Broward County. Child Welfare Case Managers work with parents, children, foster parents, group homes, the Broward Sheriffs Office, the Countys Juvenile Justice System, and many support services to meet the mission, values and goals of ChildNet and the Department of Children and Families. ChildNet provides on-site training for qualified individuals to prepare them to obtain their Florida certification as a Child Welfare Case Manager (CWCM) within a one year period.

Case Managers promotes the safety, well-being and permanency of child(ren) and their families from the time of referral until the case is closed. Actively works to do whatever needs to be done to move the child from where he or she is to the best possible outcome in the shortest possible time, fulfilling established federal and state mandates as needed by but not limited to:

  • Meet with the child(ren) and family/caregiver in their home within 48 hours and gather information about the family, including the make-up of the household, the social history of the family, any previous FAHIS reports, the educational, medical and behavioral health history of the child(ren), and the specific circumstances surrounding the alleged maltreatment. Photographs the child(ren) and arranges for fingerprinting of the child(ren) for the parents as needed by but not limited to:
  • Offer immediate practical assistance to the family in obtaining Medicaid coverage, financial assistance or help with housing.
  • Proactively develops, assesses, and maintains up-to-date case plans that accurately reflects the child(ren) and families circumstances over time. Promotes family involvement in all aspects of services to include case reviews and staffings.
  • Evaluates, prepares reports, makes recommendations and decisions on the child(ren) and families, i.e. family strengths, needs assessment, safety plan, judicial reviews, post disposition reports, status reports, home studies, and risk assessment reports.
  • Attends all court hearings and makes recommendations to the judge on the child(ren)s behalf, unless age or circumstances supersedes this requirement. Assists the child(ren) in understanding all court proceedings.
  • Interacts with families to establish needs and required services for clients through referrals for services. Works with clients to provide and refer families, caregivers, and children to the appropriate services to address identified needs. Provides support to foster homes and relative placements.
  • Coordinates monthly face-to-face visits with the child(ren) and family in the home. Observes, reports and makes recommendations based on their observations. Provides child(ren) with opportunities to visit parents, siblings, relatives, and others with court ordered visitation.
  • Initiates and reports on the reunification or alternative permanency requirements of child(ren) in accordance with Florida Statute, Chapter 39, and ChildNet-s policies and procedure.
  • Maintains and files case records in accordance with ChildNet-s policies and procedures and the states records management requirements.
  • Complies with internal tracking systems, i.e. report completion tracking, visitation logs, activity sheets and staffings.
  • Coordinates all services and other interactions with the child(ren) and family, aggressively seeks the services needed by the child and family and acts as the single point of contact through out their time in the system.
  • Responsible for maintaining contact with ChildNet staff and clients by being on call twenty-four hours a day, seven days a week via cell phone for emergencies.


Case Manager tasks involve the regular and, at times, sustained performance of moderately physically demanding work, typically involving some combination of picking-up and balancing, holding, stooping, kneeling, and crouching, and that may involve the lifting, carrying, pushing, and/or pulling of moderately heavy objects and children (20-50 pounds). May require occasional lifting, carrying, pushing, and/or pulling of heavier objects or children.

Tasks are regularly performed with potential for exposure to adverse conditions, such as dirt, dust, pollen, odors, wetness, humidity, rain, fumes, temperature and noise extremes, vibrations, electric currents, animals/wildlife, toxic/poisonous agents, disease, or pathogenic substances.
 

Skills / Requirements

An Bachelors in social work or related area of study from an accredited college or university required.

Must be available to work a flexible schedule including nights and weekends in order to meet with children and families/caregivers.

One (1) years work experience in human services or child welfare programs preferred.

Strong computer literacy including Microsoft Outlook and data entry skills required.

Ability to work with a diverse client population.

Must possess a valid Florida drivers license without any restrictions that would affect job performance.

Must have own reliable transportation and the ability to obtain and show proof of insurance.
