Medical Coordinator- Palm Beach
ChildNet, Inc. is a private, not-for-profit community-based care (CBC) agency servicing Broward and Palm Beach County. We are contracted with the State of Florida, Department of Children and Families, to provide case management support to abused, abandoned, and neglected children. ChildNet is nationally accredited by the Council on Accreditation (COA) and employs over 450 staff at its location in Ft Lauderdale and West Palm Beach, Florida.
Job Summary: This position serves as a liaison between client services, CBCIH and Sunshine Health to coordinate and schedule the medical screenings (EPSDT) and dental appointments for children in out- of home care, and in cooperation with the Child Advocates (CAS) and Dependency Case Managers (DCMs).
Makes referrals and follow-up appointments with physicians, specialists and dentists for the children based on the recommendations made during the medical screenings by:
Assisting foster parents, relatives or non-relative caregivers with medical care and dental visits, as needed.
Coordinating the transportation for children to an array of medical and dental service providers.
Verifying court documents and medical consent forms are received the day after shelter court hearings to confirm children receive their Well Child Care screening within the mandated timeframe.
Changing Medicaid plans to ensure the highest level of care for children.
Faxing prescriptions to the appropriate pharmacy and verify the prescription(s) was (were) received and picked up by the caretaker.
Transporting children to initial medical and dental appointments as needed.
Identify the network PCP for newly enrolled clients, and shall coordinate ongoing PCP changes with CBCIH.
Perform initial Health Risk Assessments for children in out of home placements.
Assist in the development of the client’s care plan.
Enter medical and dental information from medical documents email into FSFN.
Participate in utilization management integrated care team and multidisciplinary care team meetings regarding the needs of clients.
Assist current ChildNet staff as they learn the procedures regarding the scheduling of medical services for children by:
Serving as a liaison between client services, CBCIH and Sunshine Health.
Advising Child Advocates/Dependency Case Managers on complex case management issues regarding medical insurance and coverage.
Presenting at all New Hire Units and attend Unit meetings on a regular basis.
Maintaining open lines of communication with upper management, client services and medical providers to ensure each child’s medical needs are met.
Respect clients by recognizing their rights; maintaining confidentiality.
Maintain quality service by establishing and enforcing the agency’s standards.

Skills / Requirements
* Requires a high school diploma and/or higher education.
* Two (2) years experience working in a social welfare or healthcare environment preferred.
* Possess intermediate or better computer skills.
* Able and willing to work with diverse client population.
* Must have valid Florida Driver's License
